Oroville Dam stands on the Feather River in northern California and holds back a reservoir for the State Water Project. Its main concrete chute had carried water down the hillside since 1969, and [the spillways record](https://water.ca.gov/Programs/State-Water-Project/SWP-Facilities/Oroville/Oroville-Spillways/Background?ref=core-sector.ghost.io) shows it ran rarely. The chute was drawn in the early 1960s as a fifteen-inch concrete slab, laid on moderately weathered rock or better and held down by anchor bars set five feet into sound rock on a ten-foot grid. On paper it was a plain, heavy, well anchored structure, and for forty-eight years it behaved like one.Underneath ran a herringbone pattern of drain pipe, there to catch seepage and keep water from pushing up on the slab. The chute ran about ten times in fifty-one years, a few days or a few weeks at a stretch.Every stage of it passed through separate hands: a design office, a contractor, a state owner, [a federal regulator](https://ferc.gov/dam-safety-and-inspections/oroville-dam-service-spillway-p-2100?ref=core-sector.ghost.io) and an outside consultant every five years. Each one read what the one before had written.

The table below comes from one document: the assessment by [the FERC After Action Panel](https://www.ferc.gov/sites/default/files/2020-04/report.pdf?ref=core-sector.ghost.io), dated 23 November 2018, which went back through the design drawings, the construction records and eight five-year inspection reports.Every row is a dimension in inches. The row that matters is the seven. In November 1966 the drain pipe under the slab was widened from four inches to six, in the places where the ground was soft. The pipe sat on the levelled grade and the concrete went over it, which left seven inches of slab above a drain where the drawing had said fifteen. Cracks opened above those drains within a month of the pour. They were still being filled fifty years later.CASE FILEOroville Dam service spillwayFERC After Action Panel23 November 2018INCHESWHAT HAPPENEDTHE NUMBER15 inchesThe slab the drawings called for, on washed rock10 ft grid12 inchesSpacing of the one mat of bars, each way#5 bars7 inchesConcrete left above a drain once it was widened8 in short6 inchesWhat the drain pipe became in November 1966Soft ground4 inchesWhat the drain pipe was on the drawingAs drawn3 inchesCover over the bars, laid near the topOne layerA few inchesWhat the slab settled on 7 February 20171.6 feet

The panel set the drawings beside the construction record. Four things did not match.• **The anchors.** They were meant to reach five feet into sound rock. At Station 33+00 the contractor hit soft, erodible soil and priced the excavation the specification demanded. The field decision was to level the grade and pour over it, so the anchors went five feet into clay.• **The joints.** No water stop was fitted in any formed joint, and the joints sat too far apart to keep cracks narrow. Water went through the slab instead of over it.• **The steel.** One mat of bars, three inches below the top surface, where two mats would have held a cracked slab together.• **The drains.** In 1969, the first real run, flow in the herringbone drains moved in direct proportion to what was going down the chute. Infiltration speed rises with the cube of a crack width, and the cracks were widening.

On 7 February 2017 the slab settled, perhaps a few inches, and that step turned a shooting flow into a jack. Water drove through the cracks, lifted the slab and took the hillside under it.With the chute gone the reservoir kept climbing. The watershed took a full year of average runoff in fifty days. On 11 February the lake reached 901 feet and went over the uncontrolled concrete weir, 1.6 feet deep, for the first time in the project’s history. A 2005 memorandum had described that hillside as solid amphibolite that would not erode. It cut backwards toward the weir within a day.About 180,000 people below the dam were told to leave on 12 February and were home a few days later. No one was hurt. Both structures were rebuilt, and [the forensic team](https://water.ca.gov/Programs/State-Water-Project/SWP-Facilities/Oroville/Oroville-Spillways/Forensic-Team?ref=core-sector.ghost.io) set out why.

• **As drawn is not as built.** The drawing records intent. Only the construction record says what went into the ground, and there was no clear sign anyone had read it since the chute was built.• **A patch is a reading.** Cracks were filled for fifty years and the filling was logged as maintenance. The log was an instrument and it was never read as one.• **The drawing and the thing.** Every structure has two sets of drawings, the intended and the actual. A bar of gold has one. The metal is its own record.• **An instrument left in place is not an instrument.** The pressure gauges under the gate structure went erratic in 2003 and were abandoned where they sat. Nothing has measured uplift there since.I spent the late eighties on a Chicago floor where the back office ran on one man’s ledger, and it was right until the week he retired. The useful question about anything built is simple: does a record exist of what was actually installed, and when did anyone last read it?
